1.         A copy of your Permanent Resident Card (Green Card)(front and back)

2.         If you have not yet received your Form I-551, a copy of the biographic page(s) of your passport and a copy of the visa page showing your initial admission as a permanent resident;

3.         A copy of an official photo identity document showing your photo, name and date of birth. (Example: Driver's licenses, passport identity page, form I-551). The copy must clearly show the photo and identity information;

4.         Two most recent Passport Photos on a White Background of applicant;

5.         Copy of your earlier Reentry Permit;

6.         A copy of the Form I-797, Notice of Action, approval notice of an application;
