6.         Documents to certify the relationship between father and son/daughter according to the law where child lives or her/his donor father lives;

8.         Proof of legitimating for children who were born out of wedlock according to the laws of the child or father¡¯s residence or domicile (if applicable);

9.         Copy of full, final adoption decree (only required for adopted child);

10.     Death certificate of the child's US citizen parent and proof of legal guardianship (if application filed by a legal guardian in lieu of a citizen parent when parent decreased);

11.     Proof of child's US citizen father or mother's physical residence in the U.S.:

15.     Death certificate of the child¡¯s US citizen grandparent;

16.     Copy of Notice of Approval of a Form I-600(All adopted orphans applying under section 322 of the INA must either have this form or have complied with the two years legal custody and two years of joint residence requirement of INA 101(b)(1)(E));

17.     Copy of full, final adoption decree(Only required for adopted child);

18.     Evidence of all legal name changes;

If it is not possible to obtain any one of the above-required documents, you may be required to submit an original written statement from the relevant government or other authority explaining the reason for the unavailability of the document(s). You may submit the following secondary evidence for consideration:

1.         Baptismal Certificate

2.         Church Records

3.         School Record

4.         Census Records

5.         Written statements sworn to (or affirmed) by two persons who have personal knowledge of the claimed event (i.e., the date and place of a birth, marriage, or death)

Note:

Translations: Any document containing foreign language submitted to USCIS shall be accompanied by a full English language translation which the translator has certified as complete and accurate, and by the translator's certification that he or she is competent to translate from the foreign language into English;

Copies: Unless specifically required that an original document be filed with an application or petition, an ordinary legible photocopy may be submitted. Original documents submitted when not required will remain a part of the record, even if the submission was not required.
