1.         Completed "Application for Immigrant Visa and Alien Registration" (Form DS-230 Part II). Do NOT sign the form until your interview with a consular officer

2.         An Affidavit of Support (Form I-864) with sponsors' federal income tax return that was current if the affidavit of support has not been submitted to the National Visa Center

3.         Passport: Each visa applicant must have a passport valid for at least six (6) months from the time of the visa issuance, and copies of the biographic pages; AND any previous travel documents with expired and/or current U.S. visas in them

4.         Birth Certificate and copies----English translation is required if written in another language

5.         Visa Photographs: Two (2) colored photographs printed according to specifications, provided in the visa application packet

6.         Police Certificates: A police certificate is required from a country in which the applicant lived for more than one year after turning 16 years of age

7.         Certificates of trial and court transcripts if you were ever convicted of a crime

8.         Medical examinations

9.         Marriage certificates and copies

10.     Marriage Records (if applicable):

If the marriage ended by divorce, annulment, disappearance or death of the spouse, legal and/or civil documentation must be presented attesting to the termination of the marriage (annulment decree, death certificate, a foreign divorce decree, or foreign death certificate)

11.     Military Record: Applicants who have served in the military should present a certified copy of their military records

 For U.S Citizens,

1)         Your birth certificate, if you were born in the United Stated;

2)         Certificate of naturalization or citizenship issued by USCIS or former INS;

3)         Form FS-240, Report of Birth Abroad of a Citizen of the United States, issued by a U.S. Embassy or Consulate;

4)         Unexpired U.S. passport; or

5)         An original statement from a U.S. consular officer verifying that you are a U.S. citizen with a valid passport.

For Lawful permanent residents,

1)         Both sides of Form I-551, Permanent Resident Card;

2)         An unexpired temporary I-551 stamp in either a foreign passport or DHS Form I-94, Arrival/Departure Document.

3)         If you have not yet received your card, submit your passport biographic page and the page showing admission as a permanent resident, or other evidence of permanent resident status issued by USCIS or the former INS.

        Supporting Evidence of Relationship:

Parent-Child Relationship£º

1)         a copy of your birth certificate

2)         copies of evidence (such as a marriage certificate and documents showing the legal termination of all other marriages and an adoption decree) to demonstrate that you qualify as his or her child(if the other person is not your parent)

Sibling Relationship£º

1)         A copy of your and the petitioner¡¯s birth certificate

2)         copies of the marriage certificates of the father to each mother and copies of documents showing that any prior marriages of either your father or mothers were legally terminated( if have a common father but different mothers)

3)         a copy of the marriage certificate of the stepparent to the child's natural parent showing that the marriage occurred before the child's 18th birthday

4)         a copy of the adoption decree(s) showing that the adoption took place before the child became 16 years old(If are related with the petitioner by adoption)

13.     I-864 Sponsor/Petitioner¡¯s Supporting Documents

1)         Form I-864, affidavit of support

2)         Copy of unexpired US passport

3)         Copy of naturalization certificate/citizenship certification

4)         Copy of birth certificate (if you were born in the US)

5)         Copy of federal income tax return, including W-2 for the most recent tax year

Note£º Please bring all applicable documents (originals plus copies if noted) on this checklist. Visas are often approved the same day for applicants who are prepared with these documents. Visas will not be approved until applicants submit all required documents.

 

The National Visa Center sends appointment packages to the agent for applicants in certain countries when the petitions are filed in the United States. The embassy or consulate sends appointment packages to applicants in all other countries. It also sends appointment packages to all applicants whose petitions are already at the embassy or consulate.

Instruction Package & Appointment Package

Generally, Instruction Package includes:

1.         DS-2001 (Formerly known as OF-169). It is an entitled "Instructions for Immigrant Visa Applicants."

Applicants in Appointment Review Consulates must send DS-230 Part I and all the documents listed below to the NVC.

Applicants in Standard Review Consulates must send only DS-230 Part I to NVC. Rest of the documents must be carried in person to the immigrant visa interview.
